What Is In A Migraine Cocktail
The components of the migraine cocktail or rescue therapy vary considerably depending upon the emergency room or urgent care center where youre getting them. A typical rescue approach would include an analgesic and an anti-nausea agent and, in some cases, an anti-inflammatory medication, such as a steroid or something non-steroidal. Some healthcare providers will include magnesium as part of the cocktail or rescue approach.
Intravenous fluids may be useful as part of a rescue plan, especially if youre dehydrated from vomiting or poor intake.
It is generally recommended to use migraine-specific acute therapies, like a triptan, gepant, or lasmiditan, as a first line approach before going to the less migraine-specific components of a migraine cocktail or rescue therapy. Often people have tried those before they go to the urgent care or emergency department but not always.
Antiemetics Available In Australia For Migraine
*Dosages apply to adults over 18. Take these medications only as directed by your doctor or pharmacist.
None of the above medications were developed specifically for migraine, except for those sold as a combined tablet with paracetamol. Metoclopramide and prochlorperazine are the antiemetics most commonly recommended for migraine, because they dont cause major blood pressure changes, require cardiac monitoring, and may themselves also bring additional pain relief .
There is also evidence to suggest that antiemetics can work synergistically with migraine treatments particularly in the setting of nausea or vomiting . One emergency department study showed that intravenous metoclopramide and prochlorperazine had a similar level of efficacy . Your doctor will be able to recommend which drug is more suitable based on your symptoms and reaction to previous medications.
Similar to triptans, ergotamine and dihydroergotamine are migraine-specific drugs that target serotonin receptors . However, triptans have mostly replaced ergotamines, because they are more effective and ergotamines tend to cause side effects such as nausea. There are no ergotamine compounds currently available in Australia .

Your headache specialist or neurologist may be able to coordinate outpatient IV infusions for you in their infusion room if they have one to avoid a trip to the ER. Now keep in mind, these ingredients are not universally safe for everyone. The ER doctor will take your medical history and other home medications being used into account to decide which of the following medications are safe to use for you personally :
1) 2 grams Magnesium
2) 10 mg Metoclopramide or 10 mg Prochlorperazine )
3) 30 mg Ketorolac

How Should This Medicine Be Used
Sumatriptan injection comes as a solution to inject subcutaneously . It is usually used at the first sign of a migraine headache. If your symptoms improve after you use sumatriptan but return after 1 hour or longer, you may use a second dose of sumatriptan. However, if your symptoms do not improve after you use sumatriptan, do not use a second injection without talking to your doctor. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Use sumatriptan exactly as directed. Do not use more or less of it or take it more often than prescribed by your doctor.
Sumatriptan injection comes in a pre-filled auto-injection device and in vials to be used with disposable syringes. If you are using vials of sumatriptan injection, your doctor or pharmacist will tell you what type of syringe you should use. Do not use any other type of syringe because you may not get the right amount of medication.
You can inject your sumatriptan in the outer side of your thigh or upper arm. Do not inject sumatriptan through clothing. Never inject sumatriptan into a vein or muscle.
You may use your first dose of sumatriptan injection in a doctor’s office or other medical facility where you can be monitored for serious reactions. Carefully read the instructions that come with your device, and ask your doctor or pharmacist to show you how to use it.

Migraines. Migraines are a severe form of headache that can linger for several days or even weeks at a time, depending on the severity of the attack. Before the headache begins, there is a general sensation of sickness that sets in one or two days before the headache begins. Before the pain begins, some patients feel an aura, which is characterized by brilliant, flashing visual alterations.

First: Is There A Dangerous Cause
Is the headache similar to their prior headaches in character, location, magnitude, timing, associated symptoms? Are there concerning features ? Was it maximal at onset ? I find it helpful to ask: what were you doing when it started? how bad was it when it started? when was it the worst? and only after listening for a while, I backdoor into whether it is typical for them . Ive gotten myself in trouble by asking up front if its the same as usual or worst headache of your life even if they dont mean to, patients sometimes seem like they are trying to validate why they came to the ED . Of course, none of these questions are black and white and theres a lot of room for clinical judgment one minor deviation from typical headache does not mandate imaging. Patients come to see us for our expertise and often find it reassuring that weve listened and examined them and arent concerned.
Is There An Over
While a migraine cocktail usually refers to something administered by a doctor, some may use the term to describe a combination of OTC medications. Most often, an OTC migraine cocktail is a mix of three drugs:
- Acetaminophen for both pain and inflammation
- Aspirin for pain relief by reducing prostaglandins
- Caffeine to narrow blood vessels, thought to decrease discomfort
Usually, an OTC migraine cocktail consists of 250 mg of both acetaminophen and aspirin, with 65 mg of caffeine. A 2005 study found that when used together, these ingredients provide significantly better headache relief than any of the individual ingredients alone.
One example of this OTC migraine cocktail is Excedrin Migraine or Excedrin Extra Strength.

Homeopathic Treatments For Migraines
Since I am limited on how often I can use these medications, I supplement their use with homeopathic treatments. Organic muscle rubs, essential oils, magnesium oil, precision tinted glasses, herbal teas, ginger, earplugs, ice packs, moist heat and massage are also utilized. If I have an intractable migraine, that means that my normal treatments at home have not been successful in aborting the attack.
Urgent Care Or Emergency Room
When steps one and two are exhausted, the next step for me is to go to urgent care. In the past, I would dread going because there was no way of knowing if my migraine would be treated. A few years ago, my neurologist, at the time, and I came up with a treatment protocol for use in urgent care or the emergency room. When we found the right combination of medications, it was put on letterhead with a brief description of my medical history and signed by my physician.

How Does Imitrex Work For Migraines
Triptans, such as Imitrex, function by activating serotonin receptors on the walls of blood arteries in the brain, causing them to constrict. This causes the blood arteries in the brain to constrict, which alleviates the symptoms of a migraine or cluster headache. Imitrex may potentially have the additional effect of inhibiting activity in the trigeminal nerve.

What Is The Home Migraine Cocktail
If you have migraine, you absolutely must have an abortive migraine treatment. When your normal migraine abortive medications such as over the counter pain medicines, NSAIDS, triptans or gepantsdont stop the migraine, you need to have a rescue medication plan, which can be thought of as a home migraine cocktail. The goal of having a rescue or home migraine cocktail is to try to avoid the hassle of having to go to the ER all together by breaking the migraine at home.
You and your doctor should have a migraine rescue plan and either have the rescue medication available at home in case you need it, or call your doctor and request one of the following options if you have a migraine that just wont break. As with the ER migraine cocktail, your doctor will ultimately decide if these options are safe for you to use with your medical history and other medicines you may be taking.
Here are some treatments that I most commonly use for migraine rescue at home, although there are certainly other options as well:
2) Steroids are commonly prescribed for a refractory migraine attack. They need to be taken with food, preferably earlier in the day, and not with other NSAIDs. I usually use one of the following:
-Medrol dose pack
-Decadron 4 mg pill: Take 1 pill three times daily on day 1, twice daily on day 2, once on day 3
-Prednisone 10 mg: 5 pills once each morning for 5 days, then decrease by 10 mg each day until done
When Should I Go To The Er For A Migraine Attack
There’s a couple of reasons. One is if the headache is very different in its features: if there are associated neurological symptoms that are different than have been present in the past if there’s fever or if there’s an extremely abrupt onset of headache those are reasons to seek care in an emergency department to rule out emergent secondary causes of headache.
The second reason is if the symptoms are not responding to what can be delivered in the outpatient setting or given to oneself at home. With nausea and vomiting, there is an inability to deliver effective treatment because of a lack of absorption, and this results in dehydration. You would go to an emergency department to receive a therapy delivered by injection or intravenously, or if you need intravenous fluids for hydration.

Is It Possible To Overmedicate
Its tempting and easy to rely too heavily on headache medications. Some people experience daily or almost-daily headaches. For those that get migraines, the headaches may seem to blend together, with no beginning or end. You may feel the need to reach for medications to relieve symptoms or stop headaches in their tracks.
But doing so can actually make headaches worse or more frequent. In particular:
- Daily or near-daily use of relief medications can interfere with certain parts of the brain. These parts control the flow of pain messages to the nervous system. Over-the-counter medications can pose a risk, including aspirin, acetaminophen, ibuprofen, and drugs with caffeine. So can prescription medications such as triptans, narcotics and barbiturates.
- Overmedicating with abortive therapies can lower the effectiveness of preventive drugs.
Sequence And Combination Of Therapies
Stepped care within attacks is a treatment plan in which medications are added depending on patient response. Stratified care based on severity is a treatment plan in which all of the anticipated medications are given up front. Patients have improved outcomes with stratified care.
Until we have further evidence to guide our medication combinations, treating the patient with multiple medications that have independently been found to be efficacious at the onset of his or her ED visit is recommended over stepped care .

Other Medications And Tests
To ensure that the migraine is being treated aggressively, other medications are also added. For three days I am on around the clock Toradol and Thorazine and receive one dose of Solu-Medrol daily for five days. For nausea, compazine or ondansetron are given. To prevent blood clots, I am also given a daily dose of Lovenox. Dihydroergotamine effects the blood vessels in the entire body and not just the brain. Blood pressure must be monitored routinely as well as heart rate. I had a heart monitor hooked up to me the entire time. EKG’s were also done during my stay.
Does Xanax Help Migraines
Anti-inflammatories such as Celebrex, non-steroidal anti-inflammatory drugs such as Naproxen and Ibuprofen, analgesics such as Firorinal and Tylenol with Codeine, mild analgesics such as Acetaminophen and Aspirin, and stress reducers such as Tranxene, Buspar, and Xanax are all used to treat tension-type headaches.
