If I dont have aura and have migraine symptoms starting, I have a big cup of caffeinated coffee and ginger capsules if its before 2:00 pm, otherwise the caffeine will keep me up at night. If its after 2:00 I will try ginger. If things still escalate, within an hour of trying non-meds, I take Aleve. If I get any jaw pain, I know it will become a full, bad attack so I dont hesitate to take sumatriptan. Since my attacks are in better control now, sumatriptan always works on its own. Im lucky! A few years ago, when my attacks were more frequent, more intense and longer, I combined sumatriptan with Aleve and Benadryl as the attack required a bigger punch to knock it out.

  • Valproate: This anti-seizure drug is sometimes ordered to prevent recurrent migraines. Valproate is considered highly teratogenic, and is banned in many countries for use in pregnant women and women of childbearing age.

What If A Treatment Doesnt Work

Treatment failure is often a product of miscommunication and misunderstanding, perhaps about a persons symptoms or about what someone actually needs to treat their specific type of migraine. In other words, it may not actually be a failure at all.

A key part of understanding treatment failure, said Lipton, is redefining success. A preventive treatment plan will inevitably lead to disappointment if the expectation is to never have another headache. Instead, Lipton said, a reasonable goal for treatment is to decrease a persons headache days by 50% or 75% or reducing the intensity of the headaches by 50%-75% and allowing acute medications to work faster and more effectively. This will minimize the time that headache is interfering with ability to function.

Other Medications And Tests

To ensure that the migraine is being treated aggressively, other medications are also added. For three days I am on around the clock Toradol and Thorazine and receive one dose of Solu-Medrol daily for five days. For nausea, compazine or ondansetron are given. To prevent blood clots, I am also given a daily dose of Lovenox. Dihydroergotamine effects the blood vessels in the entire body and not just the brain. Blood pressure must be monitored routinely as well as heart rate. I had a heart monitor hooked up to me the entire time. EKG’s were also done during my stay.
