  • Viral Illnesses. Most acute headaches are part of a viral illness. Flu is a common example. These headaches may relate to the level of fever. Most often, they last a few days.

  • MSG Headache. MSG is a flavor enhancer sometimes added to soups or other foods. In larger amounts, it can cause the sudden onset of a throbbing headache. Flushing of the face also occurs.
  • Common Harmless Causes. Hard exercise, bright sunlight, blowing a wind instrument or gum chewing have been reported. So has severe coughing. “Ice cream headaches” are triggered by any icy food or drink. The worse pain is between the eyes .
  • Head Injury. Most just cause a scalp injury. This leads to a painful spot on the scalp for a few days. Severe, deeper or entire-head pain needs to be seen.
  • Frontal Sinus Infection. Can cause a headache on the forehead just above the eyebrow. Other symptoms are nasal congestion and postnasal drip. Rare before 10 years old. Reason: the frontal sinus is not yet formed. Other sinus infections cause face pain, not headaches.
  • Meningitis . A bacterial infection of the membrane that covers the spinal cord and brain. The main symptoms are a stiff neck, headache, confusion and fever. Younger children are lethargic or so irritable that they can’t be consoled. If not treated early, child can suffer brain damage.

Not just a headache” has become a rallying cry for the millions of us living with Migraine. Even though Migraine creeps into almost all areas of our bodies and our lives, the general public still thinks of it as a “headache,” which is ironic when nausea and vomiting are among the most common symptoms.

Dr. Amaal Starling, headache specialist and Assistant Professor of Neurology at the Mayo Clinic, is just one of many valuable people in the Migraine community working hard to educate doctors, patients, and the public. Her education, clinical expertise, and personal experience provide her with a valuable perspective on Migraine disease.

We sat down with Dr. Starling to find out why so many of us experience a stomach ache with Migraine attacks.

What Causes Headache And Nausea

Migraine headaches are a common cause of combined headache and nausea. Migraines can cause a variety of symptoms, including nausea, dizziness, sensitivity to light, and severe headache pain. Theyre often preceded by a visual or sensory disturbance, called an aura.

Other conditions associated with headache and nausea include dehydration and low blood sugar. Dehydration can occur when you dont drink enough fluid.

Low blood sugar can develop for a variety of reasons, including excessive alcohol consumption, medication side effect, severe liver or kidney disease, long-term starvation, and hormonal deficiencies. If you have diabetes, taking too much insulin can also cause low blood sugar.

Other conditions that can lead to headache and nausea include:

  • neck stiffness and a fever
  • vomiting for more than 24 hours
  • no urination for eight hours or more
  • loss of consciousness

If you suspect you need urgent care, seek help. Its better to be safe than sorry.

What Happens During A Migraine

Every migraine begins differently. Sometimes people get a warning that a migraine is on its way. A few hours or even days before the actual headache, people might feel funny or “not right. They might crave different foods, or feel thirsty, irritable, tired, or even full of energy. This is called a “premonition.”

Some people get auras. These are neurological symptoms that start just before the headache and last up to an hour. An aura is different in every person, but it often affects vision. For example, a person might:

  • have blurred vision
  • see spots, colored balls, jagged lines, or bright flashing lights
  • smell a certain odor
  • feel tingling in a part of their face

Once the headache starts, light, smell, or sound may bother people with migraines or make them feel worse. Sometimes, if they try to continue with their usual routine, they may become nauseated and vomit. Often the pain begins only on one side of the head, but it might eventually affect both sides. Trying to do physical activities can make the pain worse.

Most migraines last from 30 minutes to several hours; some can last a couple of days.

Heading home from an office party in Stillwater, Okla., Angie Mendez became the main character in a frightening medical mystery. Suddenly, she felt so sick that she wasnt sure she could drive the last few blocks to her house. Chills, nausea and stomach cramps hit with little warning, like a lightning strike, she said. She barely made it into the house before explosive vomiting and diarrhea started. Her temperature spiked to 102. Her back ached.

Ms. Mendez, the marketing director for a restaurant and clothing store, is the subject of an article published Wednesday in The New England Journal of Medicine by Dr. James N. George and other researchers. She was not named in the article, but was contacted through Dr. George, and agreed to be interviewed. Her illness occurred in 2009.

Still sick and in pain the morning after she first became ill, she went to an emergency room. There were hints that something might be seriously wrong. Her blood counts were abnormal: White cells were high, platelets a bit low. She couldnt produce a urine sample. But she was 35 and healthy, and her main symptoms matched those caused by run-of-the-mill stomach bugs. Doctors gave her nausea medicine and intravenous fluid, and sent her home.

Two days later, she was back in the emergency room. Now, her body was clearly under siege. She had not urinated in days, not since the illness began. Something had shut down her kidneys.

Recovery Or Postdrome Stage

This is the final stage of an attack, and it can take hours or days for a drained, fatigued or hangover type feeling to disappear. Symptoms can be similar to those of the first stage . Often, they mirror these symptoms. For example, if you lost your appetite at the beginning of the attack, you might be very hungry now. If you were tired, you might feel full of energy.

Being aware of the different stages of the migraine attack can be helpful. It can help you prepare for an attack, get a diagnosis and decide when to take acute treatment, such as painkillers or adapt your activities.

It is useful to have a rescue treatment plan for when attacks occur. This may include painkillers such as a triptan, a NSAID or paracetamol. It often also includes anti-sickness medication.

For other people, being aware of the stages and symptoms of a migraine attack can help their understanding. It may also help with the frustration and lack of understanding people often face around migraine, especially at work and in education.

A migraine is a common neurological disease that causes a variety of symptoms, most notably a throbbing, pulsing headache on one side of your head. Your migraine will likely get worse with physical activity, lights, sounds or smells. It may last at least four hours or even days. About 12% of Americans have this genetic disorder. Research shows that its the sixth most disabling disease in the world.

Histamine Can Cause Stomach Upset

When histamine is released in the gastrointestinal tract, it can cause symptoms like stomach pain, bloating, nausea, diarrhea, and constipation.

Underlying conditions and imbalances can also cause too much histamine to be released.

A few examples are small intestinal bacterial overgrowth , underlying or low-grade infection, poor diet, heavy metal toxicity, nutrient deficiencies, and food sensitivities.

How Are Migraines Treated

Migraine headaches are chronic. They cant be cured, but they can be managed and possibly improved. There are two main treatment approaches that use medications: abortive and preventive.

  • Abortive medications are most effective when you use them at the first sign of a migraine. Take them while the pain is mild. By possibly stopping the headache process, abortive medications help stop or decrease your migraine symptoms, including pain, nausea, light sensitivity, etc. Some abortive medications work by constricting your blood vessels, bringing them back to normal and relieving the throbbing pain.
  • Preventive medications may be prescribed when your headaches are severe, occur more than four times a month and are significantly interfering with your normal activities. Preventive medications reduce the frequency and severity of the headaches. Medications are generally taken on a regular, daily basis to help prevent migraines.

Breathing Or Respiratory Issues

Many people suffer from breathing and respiratory issues when they sit in a room with an air conditioner and feel better when away. This often happens when the air conditioner and especially its air filters are not cleaned or changed when needed.

Paula: Can you explain cyclical vomiting and abdominal migraines?

Dr. Starling: Cyclic vomiting syndrome and abdominal migraine are variants of Migraine, and they’re actually more common in our pediatric population. With abdominal migraine, individuals will have episodes of abdominal pain that is usually poorly localized somewhere in the stomach area.

These episodes will come and go, similar to migraine attacks that will come and go. There are patients for whom abdominal Migraine develops into a more chronic problem similar to Chronic Migraine.

Cyclic vomiting syndrome is another Migraine variant where they’ll have episodes where they’re unable to stop vomiting, and then episodes where they’re doing okay in between – similar to having episodic Migraine. Both of these things, as I mentioned, are more common in the pediatric population, but they can be treated effectively with the same types of medications that we use for Migraine, whether they have a headache or not.

That is another important concept: Migraine should not be synonymous to head pain. Migraine is a sensory processing disorder, and so some people have Migraine without any head pain at all, and they may only have nausea, the abdominal pain, all of these other symptoms that talk about.

Migraine Without Head Pain

Also called a Silent or Acephalgic Migraine, this type of migraine can be very alarming as you experience dizzying aura and other visual disturbances, nausea, and other phases of migraine, but no head pain. It can be triggered by any of a persons regular triggers, and those who get them are likely to experience other types of migraine, too. The International Headache Society classifies this type as typical aura without headache.

Youre Having Trouble Thinking Clearly

If youve been having a tough time concentrating on one task at a time, remembering things accurately, or just generally operating on a higher level, stress could be to blame.

This mental fatigue sometimes happens when small stressors pile in at a volume we cant keep up with. Things like making multiple tough decisions at work, handling ongoing interruptions, and juggling social commitments all these can accumulate and start to weigh on you. If you don’t have a chance to hit the pause button and reset, brain fog could set in.

The unfortunate reality of this mental fatigue is that it can affect your physical energy levels, too.
